1. TEAM NAME: Think of something creative!
2. TEAM CAPTAIN: Please include email and phone number
3. TEAM MEMBERS: Min. of 2 up to 4 players per team-Individuals are welcome to sign up, we’ll put you on a team!
4. SKILL LEVEL: A. Top of the League, B. We really love Volleyball or C. Just for Fun!! (must be age 16+ to play)
